Please arrange a referral from your GP or treating clinician and contact reception to book. Tell reception which consultation or test is requested. An enquiry or email does not confirm an appointment; reception will confirm the booking.
Bring your referral, Medicare card, current medicine list or medicines, relevant blood tests and previous cardiac results. Tell reception about allergies, including reactions to adhesive patches, and any recent hospital attendance.
See the service information for draft preparation guidance. Reception will provide confirmed instructions for your specific test. Do not stop or change medicines without advice from your treating clinician.
Current consultation fees, test fees, Medicare eligibility and any out-of-pocket costs must be confirmed by reception before your appointment. Draft payment policy: card/EFTPOS, payment by telephone or bank transfer; no cash accepted or kept on the premises. Please send confirmation for a bank transfer. This policy requires final practice approval.
Please give at least 48 hours’ notice if you need to cancel or reschedule. Contact reception by telephone. Final cancellation terms and any applicable charges require practice confirmation.
Please tell reception your preferred language and dialect, and whether you need a professional interpreter. Reception will confirm availability and any charges before arranging the service. Please also tell us about hearing, vision or communication needs.
Please let reception know about mobility difficulties, walking aids or other adjustments you need. Discuss your ability to walk on a treadmill before booking exercise testing. Assistance animals are welcome. Step-free access, accessible parking and toilet details are awaiting confirmation.
Ask reception whether telehealth is suitable and available for your appointment. Connection details and any telephone alternative will be confirmed by the practice.
Read online or download the supplied practice documents. Clinical instructions remain for Dr Khan’s approval before public launch.
